The mechanism by which biguanides increas e the activity of AMPK remains uncertain; however, research suggests that metformin increases the amount of cytosolic AMP (as opposed to a change in total AMP or total AMP/ATP).[6] In addition to suppressing hepatic glucose production, metformin increases insulin sensitivity, enhances peripheral glucose uptake, decreases fatty acid oxidation, and decreases absorption of glucose from the gastrointestinal tract.[8] Increased peripheral utilization of glucose may be due to improved insulin binding to insulin receptors.[9] AMPK probably also plays a role, as metformin administration increases AMPK activity in skeletal muscle.[10] AMPK is known to cause GLUT4 translocation, resulting in insulin-independent glucose uptake. Some metabolic actions of metformin do appear to occur by AMPK-independent mechanisms; a recent study found that the metabolic actions of metformin in the heart muscle can occur independent of changes in AMPK activity and may be mediated by p38 MAPK- and PKC-dependent mechanisms.[11] Metformin causes a few gastrointestinal side effects including nausea, metallic taste, diarrhea and abdominal discomfort[7] . These can be avoided if the dose is increased slowly, and taking the drug with meals. A small amount of weight loss, possibly due to drop in net caloric intake due to appetite repression and/or a reduction in hyperinsulinemia is suggested. Falling in the same drug class as phenformin, the reported incidence of lactic acidosis is surprisingly low, 0.03 per 1000. In a US double-blind clinical study of GLUCOPHAGE in patients with type 2 diabetes, a total of 141 patients received GLUCOPHAGE therapy (up to 2550 mg per day) and 145 patients received placebo. Most Common Adverse Reactions (>5.0 Percent) in a Placebo-Controlled Clinical Study of GLUCOPHAGE Monotherapy The occurrence can further be avoided if contraindications are followed. It is contraindicated in people with a high risk of lactic acidosis: renal serum creatinine levels over 150 ÃŽ ¼mol/l[14}or hepatic impairment, respiratory insufficiency, severe infection and alcohol abuse. Any pharmacological therapy that alters either of the factors mentioned before is also considered. It should also be used cautiously in elderly especially those above 80 years of age. It is recommended to monitor renal function upon initiation and at least once a year thereafter. It should be withheld immediately before a person has a procedure with a radiocontrast dye, as the dye increases the risk of renal failure and therefore lactic acidosis [15] [16]. It should also be discontinued before and surgery and can be started immediately after if the renal function is normal and the patient is stable. It is also recommended to monitor hematological parameters as it alters vitamin B12 absorption [12] [13] and therefore cause anemia (7% in clinical trials). The Stamp Act of 1765 was a tax put on the British American colonies, sponsered by George Grenville and was the first direct tax placed on them. Parliament needed means to help fund expensive costs of keeping troops inside the colonies, so they imposed a tax on all of the colonies everyday printed materials, such as pamphlets and newspapers, and all legal and commercial documents, which all needed to have a certain special stamp placed on it. Many agents of the American colonies that resided in London had suggested other methods to obtain the needed money but where ignored and the Americans where taxed. There were many Americans who did not elect members of Parliament and they greatly opposed the Stamp Act, for two reasons, because they didn't have enough money to pay for them and also it went against their new principle that said, " No taxation without representation." This new tax made many Americans very angered and their actions that came from this started the way towards the American Revolution. There was many forms of resistance, including the king and Parliament receiving many petitions, Americans boycotting the British goods, and printers and lawyers refusing to use the stamps for stamping documents. Another major point is that violence sparked up from the Sons of Liberty, and in New York rioters got so violent that the destroyed a British officer's house because he had said that he "would cram the stamps down America throats at the point of his sword." Some believe there was a conflict of interest.It is also believed Andersen was influenced to destroy auditing documents because of the large consulting fees Enron paid them. Only one female deputy, Deputy Cynthia Hall, escorted the uncuffed, 6-foot-1, 200 pound, accused rapist, w ho was just caught the day before with homemade “shanks” in his shoes, to the courtroom (Dewan, 2005). Law enforcement definitely made an error in judgement when they decided to let one deputy escort the defendant. Even though the law prohibited them from cuffing Brian Nichols as he entered the courtroom, they definitely could have upped their security of him a lot more. I understand the point about avoiding prejudicing judges and juries, but at the same time I think innocent peoples safety should be the number one priority. Of course the other legal issue here is cold-blooded murder, yet this is not really an “issue” because there were so many witnesses that Brian Nichols would definitely not beat this one in court.A civil suit definitely could be filed by the judges family and by the court reporters family in this case. It could be brought against the Fulton County Police Department for failing to take appropriate precautions that would ensure the safety and well-b eing of everyone in that building.Brian Nichols avoided the death penalty when a jury deciding his fate deadlocked after four days of deliberation. The jury was split 9-3 in favor of giving Nichols the death penalty rather than life in prison (Montaldo, 2008). Superior Court Judge James Bodiford had only two options in sentencing Nichols, life without parole or life with the possibility of parole.
